TYPE 2 DIABETES PROGRAMS
Sansum Diabetes Research Institute understands that whether newly diagnosed or living with diabetes for a long period of time, it is an overwhelming burden. Proper support is critical in one’s successful management of diabetes. SDRI provides classes in both English and Spanish ranging from nutrition, exercise, and healthy living. SDRI further provides a variety of outreach programs for children, adults and seniors impacted by type 2 diabetes.
CURRENT TYPE 2 DIABETES PROGRAMS
Mobile Health Station
The Mobile Health Station is a free diabetes screening and risk assessment program offered at accessible community locations. Participants receive A1c testing and measurements of their blood pressure, waist circumference, and BMI.
Following these measurements, individuals have the opportunity to review their results in a one on one consultation with an experienced diabetes educator. Resources for continued care and free classes in diabetes and nutrition are also provided.
Diabetes Prevention Program
The Diabetes Prevention Program for Seniors is a community-based self-management program developed by the Center for Disease Control, and designed to prevent or delay the progression from pre-diabetes to type 2 diabetes. The program gives participants the skills and information needed to manage their health and stay active.
Nutrition & Cooking
Our weekly Nutrition Classes are a resource for individuals with type 2 diabetes and pre-diabetes to help identify changes that can be made to improve diabetes self-care. Our cooking classes are held monthly and promote healthy cooking for diabetes through a cooking demonstration, interactive conversation, and new recipes. Classes are offered in English and Spanish.
Ocho Pasos a la Buena Salud
Ocho Pasos a la Buena Salud is an 8-week diabetes self-management education series. Topics include healthy eating, meal planning, and preparation, as well as identifying risk factors, symptoms, and complications of diabetes. Health outcomes are tracked over the course of the series and have proven the effectiveness of the Ocho Pasos program. Classes are offered in Spanish.
Semillas de Cambio
Semillas de Cambio is a 6-week educational program for Latina women who have experienced a pregnancy complicated by diabetes, or are at risk of developing diabetes in the future. Classes are taught by trained community health educators who provide a safe learning environment focused on improving health before a subsequent pregnancy. The program is cost-effective and able to be replicated, with curriculum covering nutrition, exercise, and other diabetes relevant themes. Participants learn tools to create lasting change, while also encouraging and supporting one another.
Promotores de Salud Training Program
Our Promotores de Salud (Community Health Workers) Training Program provides professional development opportunities for Promotores, to equip them with the skills necessary to engage the local Spanish speaking community in culturally relevant classes to improve health awareness and implement positive change in their own lives. This program is currently in the process of being formally certified by the AADE.
